New Patient Intake
Central Penn Wound Care works closely with your healthcare team, including home health providers, to focus on healing your wound and improving your quality of life. We strive to make the referral and new patient process as simple as possible, ensuring patients are scheduled within a week.
While we accept most insurance plans, the majority of our patients have Medicare coverage. A medical referral is not required, but the following information is essential for processing referrals.
PLEASE NOTE: OUR NEW PATIENT REFERRAL FORM MUST BE COMPLETED IN FULL TO INCLUDE ALL WOUND INFORMATION.
Referral Guidelines
Indications to refer a patient:

Non-healing wounds that are 4 weeks old.

Open wounds measuring 1cm or greater.

Chronic wounds with exposed nerve, tendon, bone, or muscle.

Non-healing venous stasis ulcers, diabetic foot ulcers, arterial ulcers, neuropathic ulcers, pressure injuries, vasculitic ulcers, scleroderma ulcers, and surgical or traumatic wounds.
How to Refer a Patient

Complete the New Patient Referral Form
The online form is submitted directly to our office via a secure, HIPAA-compliant system. If completing the form online is not possible, contact our office to request a printable version by email or fax. -
Provide Supporting Documents
Include the latest progress notes, current medications, and any relevant laboratory results. -
Submit Supporting Documents
Upload documents securely using the Contact Form below, or send them via our HIPAA-compliant email or fax.
Once authorization is approved (typically within 48 hours), we will contact your patient to schedule their first appointment.
